Please note: If you do not have a scanner, you MAY take a photo of your model release form for submission, however IF IT IS NOT CLEAR enough to read, you will be required to retake the picture or scan it if you cannot manage to get a clear picture of the form.

Your picture of you holding your photo I.D. next to your face is for identification and age verification purposes only - D.O.B. should be clear and legible in photo. You MAY send an extra photo of JUST your photo I.D. if you cannot manage to get a clear picture of your D.O.B. when it is held up to your face.
